How to Request a Place on a Course

All of our training courses are free to organisations based in Derbyshire.  You can request a place on a training course by email, post or phone.  To check the dates the courses are running click on the icon for Current Academic Training Course Dates or download a copy of the Training Brochure.

To request a place on a course through the website by clicking on the icon to view the Current Academic Training Course Dates. Select the date of the course you wish to attend, this will generate form which you will need to complete before sending. 

Print off a copy of the Training Course Booking Form, complete the information requested and post it to Penny Elks, Derbyshire Health Promoting Schools at Derbyshire County PCT, Babington Hospital,  Derby Road, Belper, DE56 1WH (Address is also listed at the bottom of the form).
