How to Request a Place on a Course

All of our training courses are FREE to organisations based in Derbyshire.  You can request a place on a training course by email, post or phone.  To check the dates the courses are running download a copy of the Training Brochure.


Post: 

Print off a copy of the Training Course Booking Form, complete the information requested and post it to Penny Elks, Derbyshire Healthy Schools at Derbyshire County PCT, Babington Hospital,  Derby Road, Belper, DE56 1WH (Address is also listed at the bottom of the form).
